I've just tested these rooms again and found that if Jack loses a life in either room 15113 or 15114, even if he hasn't pulled a lever, he is still stuck.
If he loses a life in 15113 the blue bat platform doesn't re-appear so he has no way to move to room 15114.
If he doesn't lose a life in 15113 and he moves successfully to 15114 and then immediately loses a life there he re-appears back in 15113, again with no bat platform.

My Jack did loose his life but when he re-appeared all went well. So I would never have found that problem.
